How ScanTrace Works
Access and Login to your employee portal via web link.
Fill out your Health Declaration Form (HDF) and then click submit.
NOTE: HDF should be filled out daily.
Fill out your Case Investigation Form (CIF).
NOTE: Fill this out before swab test is to be undertaken.
Helpful Tip: If there’s no printed ID, you may easily access the assigned QR CODE by:

on your smartphone.
Printing a copy of your QR CODE
on a paper.
